Reducing avoidable acute care hospitalizations is a complex challenge that crosses all healthcare provider settings. The Visiting Nurse Association of Boston is taking the lead to address this costly issue. We have implemented Best Practice Tools to prevent avoidable hospitalizations for patients receiving homecare services.
VNAB’s ReACH program is producing results through the use of outcome based quality improvement methods and integrating health information technology and treatment. We have redesigned care processes and systems to support our mission to reduce avoidable hospitalization.
