Community Resource Specialists (CRS) at the VNA of Boston know and understand all of the community resources our patients need and how to access them.
- ELDER SERVICES: help patients access basic support services such as: homemakers, Personal Emergency Response Systems (PERS), Meals on Wheels, Adult Day Health, etc.
- TRANSPORTATION: Efficient and safe arrangements are made to get patients to their medical appointments. Transportation can also be arranged for adult day settings, places of worship, pharmacies, shopping, etc.
- ENTITLEMENT PROGRAMS: Isolated seniors often lack family or friend networks, access to information, and the ability to take advantage of programs that can help them such as: MassHealth, Food Stamps, Social Security, fuel assistance, Medicare Part D, etc.
- SPECIAL POPULATIONS: CRSs are a crucial link to resources and societies.
- Mothers and Infants (Cradles to Crayons, support groups, WIC, etc.)
- Chronic Illness (MS, ALS, ALZ, etc. support groups and societies)
- Local disability agencies – Commission for the Blind, Commission for the Deaf and Hard of Hearing, Department of Mental Health, etc.
